Giving children the future they deserve with funding for critical healthcare in Southwest Missouri
Through our grantmaking process, we work to ensure that any child who needs mental, dental, or physical healthcare in the Ozarks can receive quality attention and treatment. We aim to ease the financial burden of chronic and specialist healthcare on young people in need. Now, nonprofit agencies that focus on children’s needs can apply for funding assistance online in just a few steps.
Become a grant recipient in 3 steps
Step 1: Grant concept letter
Submit a one-page concept letter during one of two grant cycles, describing the critical need to be addressed, the amount requested, and contact information. Also, include a preliminary budget for the grant request. Concept letters are due the last working day in September for February awards and by the last day in March for July awards. Submit your concept letter.
Step 2: Grant application form
We’ll notify you if your concept letter is denied or approved. On approval, you can submit a completed grant application form by the deadline in your approval letter. You’ll need the following to complete your application.
- Mission statement and board members and their addresses
- Your organization’s 501(c)(3) letter or equivalent
- Your most recent 990 tax return form
- Most current income statement and balance sheet
- Most recent audit. Audit waivers will have to be approved by the OHAF Board of Directors.
Step 3: Receiving your grant awards
Congratulations! Your grant application has been approved. You will receive a check in the mail. Being able to award our beneficiaries with grants is extremely fulfilling. We know the joy and sense of relief that comes from receiving healthcare funding because we know how hard it can be to access it in today’s trying times.
Giving feedback
You must submit a progress report and receipts approximately one year after your grant award to tell us how you used your funds. If you fail to submit the progress report and receipts, you will not be eligible to apply for a grant for three cycles.
Am I eligible?
If your organization meets all the requirements listed below, you are eligible to submit a concept letter.
- 1. Organizations must have a current 501 (c)(3) Public Charity Status from the Internal Revenue Service.
- 2. Organizations must have a presence in one of the following Southwest Missouri counties: Barry, Barton, Cedar, Christian, Dade, Dallas, Greene, Hickory, Jasper, Laclede, Lawrence, McDonald, Newton, Polk, St. Clair, Stone, Taney, Webster, and Wright.
- 3. Organizations must not have any delinquent final reports from previous grants.
- 4. Organizations are not eligible to receive grant funds in two consecutive cycles.
OHAF funding requirements
- Grants are accepted for children’s critical medical, mental, and dental health-related needs in Southwest Missouri (See counties above).
- Concept letters must be submitted by the deadline for consideration.
- Grant applications will only be considered following concept letter review and approval.
- Periodically, the Foundation may grant funding based on urgent needs identified by community-based needs assessment efforts.
